| In order to help you get exactly what you want when you request your quote for a CJT mounting system, CJT has made available to you the Helping Hand Worksheet and the Photo Tip Sheet . The Helping Hand Worksheet is a Microsoft Word 2003 template that can be filled out on your computer. It has specific questions about the wheelchair on to which the mount will go. The Photo Tip Sheet is a brief instruction sheet on how to take pictures that present the most helpful information for evaluating a wheelchair for a mounting system. We recommend that you save the Helping Hand Worksheet template file to your computer to fill out. After you have filled the worksheet out, save the template with the information using a file name that references your client and wheelchair. Then email the completed Helping Hand Worksheet file and photos to quoterequest@yescjt.com. Note that most email services will not allow attachments greater than 26 megabytes. So it maybe necessary to send multiple emails with attachments smaller than 26 megabytes. If your version of Microsoft word is older than 2003 please print out the PDF version and fax the completed form to CJT at 714-751-5775. |
